Our Ryan White Medical Case Management Program provides coordinated primary and preventative care and social services to individuals living with HIV/AIDS. The Program provides education to patients, including preventing the transmission of the virus to others. In addition, the Program gives ongoing coaching support to help HIV patients become self-sufficient and active participants in managing their care.
The Ryan White Program is available at our WCHC Mount Vernon location.

How the Ryan White Medical Case Management Program Works
All patients living with HIV/AIDS are eligible to enroll in our Ryan White Program. Once enrolled, each patient is assigned a Care Coordinator and a Patient Navigator to help integrate their medical care and services throughout the Health Center and obtain social services if needed.
Our Care Coordinators work closely with their patients’ medical teams and Certified HIV Specialists to oversee their patients’ care plans and help them communicate their needs. They assess how well their patients are keeping up with their medications and antiretroviral therapy. If support is needed, the Care Coordinator provides treatment adherence education and support to help remove any care obstacles. The Coordinators also identify and enroll their patients in any social services they might need. Services might include housing or rent subsidies, entitlement programs such as food stamps (SNAP), AIDS Drug Assistance Program, or Emergency Assistance.

The Patient Navigator can also help to schedule appointments and even accompany patients to their health care appointments.
Worried you might be HIV+?
HIV testing is available to all our registered patients. Please ask your primary care physician to be tested. State law protects the confidentiality (privacy) of your HIV test results. It also protects you against discrimination based on your HIV status.
If you test positive, you will be referred to our Ryan White Medical Case Management Program immediately to be provided with coordinated primary, preventative, and social services.
