Patient Advocacy Service

Your patient advocate helps you communicate with your healthcare provider. They can guide and connect you to services and supports that will help you and your family stay healthy.

340B Low-Cost Prescription Plan

Discounted Prescriptions (340B Program)

As our patient, you are eligible to receive your prescriptions at discounts of up to 70% off at participating pharmacies through the 340B Drug Discount Pricing Program.

Transportation for Patients to their appointments

Patient Transportation

All our locations offer free patient transportation services to make it easier for you to get to your appointments. We have on hand 8 vehicles to provide non-emergency patient transportation.

WIC Program

Are you expecting a baby or have a young family? You may be eligible to receive FREE nutritious food along with other valuable benefits and services through the WIC Program.

PICHC Program

Through the Perinatal and Infant Community Health Collaborative (PICHC), pregnant and new mothers can get the healthcare they need and be connected to services and programs.

Food Programs and Pantries

Our network has a number of programs that it runs to address food deficits in our communities.

Delivering healthcare into the community

Mobile Health

Our mobile van frequently visits churches, housing complexes, parks, health fairs, and other places where the community gathers to deliver healthcare services and education.

Ryan White HIV/AIDs Care & Support

Ryan White HIV/AIDs Care & Support

Our Ryan White Medical Case Management Program provides patient-centered, comprehensive medical services, and support services to improve the health outcomes for people living with HIV.

On-Site Pharmacy

We are pleased to offer to WCHC Mount Vernon patients a convenient and affordable alternative for meeting their prescription and personal care items needs.

We are pleased to offer on-site independently administered and operated laboratory testing services at our Mount Vernon, Greenburgh, and Yonkers locations.
