Along with proper diet, medication, and exercise, regularly monitoring your blood pressure and glucose levels is essential for managing chronic hypertension and diabetes.
Our Remote Patient Monitoring Service helps patients who could benefit from ongoing monitoring achieve their health goals and stay connected to their care team. Program participants are given a monitoring device that they can use at home. This home device directly connects to their health file on our systems so that our medical providers can have a continuous record of readings and attend to patients’ needs in real-time.
Eligibility
Your medical Provider determines whether you are eligible for remote patient monitoring based on your medical history. Patients are generally eligible if their HA1C level is consistently over 9 or their blood pressure is 140/85 mm/Hg or higher.
For us to set up and receive digital communication from your device, you must have access to a smartphone (e.g., an iPhone, Samsung Galaxy, Jitterbug), a tablet, a laptop, or a desktop computer.
How to Remotely Record your Blood Pressure or Glucose Levels
Once your Provider determines that you are eligible for remote patient monitoring, we will contact you to schedule an appointment to get your device. At the appointment, you will sign a consent form, pick up your device, and receive instructions on how to get accurate readings at home.
