Helping you manage chronic health conditions
Managing many chronic health conditions such as hypertension, diabetes, and HIV often requires additional health care services and professionals. We develop a customized care plan for you based on your particular health condition and its severity. In addition to regular medical checkups and medication, our internal medicine professionals provide referrals to specialists, both inside our system and outside, and diagnostic testing to monitor your progress.

Asthma
Asthma is a chronic lung condition that makes it difficult to temporarily breathe. We treat asthma with medication such as inhalers and help to identify triggers that can help you manage your symptoms.

High Blood Pressure
Hypertension (high blood pressure) is when the heart has to work too hard to move blood around the body. We prescribe medications, recommend lifestyle changes, and, if needed, refer patients to more specialty care and services.

Diabetes
Diabetes is a condition where the body cannot regulate how much glucose (sugar) is in the blood. We treat diabetes with medications, suggest lifestyle changes, and offer referrals as needed to in-house specialists.

HIV/AIDS
HIV is a virus that attacks the body’s immune system that, left untreated, can turn into AIDS. Our Ryan White Medical Case Management Program provides care and social services to individuals living with HIV/AIDS.

We understand. We aim to be as flexible as we can to accommodate your ability to pay and to provide you with the care that you need.
Our center accepts a range of health insurance plans including Child Health Plus, Medicare, and Medicaid. If you don’t have health insurance, we can help you find and enroll in an affordable plan that meets your needs and budget. For individuals or families who do not qualify for health insurance, we offer a sliding fee program for essential services as well as payment plans.
