Our mission is to provide quality, affordable primary and preventative health care to all members of our community. We aim to be as flexible as we can to accommodate individuals and families with varying abilities to pay. We will work with you so you are comfortable with paying for care. We do not turn away anyone due to inability to pay or immigration status.

Don’t have health insurance? We have options.

We can help you enroll in an affordable health insurance plan.

We have Certified Application Counselors available at select locations to help you enroll in a health insurance plan that fits your needs and budget. Our Counselors can determine whether you are eligible for state-funded assistance, including Medicaid or other New York State Programs. If you qualify for health insurance, they can recommend affordable and comprehensive health insurance options available through the New York State of Health Marketplace. Our Counselors can also assist you in filling out and processing your application.

Patient Sign Up Health Insurance

We offer sliding fees for services.

We have a sliding fee program for essential services for patients who do not qualify for health insurance. Essential services include visits to medical doctors and dentists; laboratory testing; x-ray and ultrasound; and 340B plan pharmacy discounts. As our testing laboratories are outside companies, you will be asked to pay for laboratory testing at the time the test is being done or request to be invoiced. For those patients who cannot pay the full amount of their sliding fees, we can offer payment plans.

All patients, regardless of health insurance coverage, are eligible to be considered for a sliding fee co-pay discount.

Essential services and co-pay sliding discounts are based on the proof of income that a patient presents at registration or re-certification. Recertification happens when there is a change in the Federal Poverty Guidelines, which are issued by the U.S. Government once a year. Documents that satisfy proof of income are listed in the New Patient Registration Document section of the Prepare for a Visit page.

If there is a change in income and/or family size, patients can update their information at any time by contacting the registration department.
