Home | For Patients | Our Care Philosophy

Our focus is on you.

We provide personalized care tailored to your unique health concerns, stage of life, and daily challenges. In addition to providing quality primary care and specialty care, we offer a range of supporting services to help keep you healthy and your health care affordable and accessible.

Father and Son

We are your Medical Home.

Your place to turn to for patient-centered, quality, and coordinated health care.

Your place where we partner with you to ensure your good health.

What is Patient-Centered, Coordinated Care?

Your medical care may involve several health professionals and services. Patient-centered, coordinated care involves your health care team working together with you to achieve your health goals.

Your primary care physician heads your health care team. He/she coordinates your care across our centers and with various departments and professionals.

At the time of registration, all our patients select a primary care physician to oversee their care. Your primary care physician is responsible for creating a personalized care plan with you and explaining the plan to you in a manner that you can understand. Your care might involve other medical professionals and support services, which your primary care physician will then oversee and coordinate. If you need care outside our centers or need to be admitted to a hospital, your primary care physician will be actively involved in monitoring your progress.

Working together, everyone on your health care team shares the same information and is focused on helping you achieve your healthcare goals.

Patient-Centered Care involves working as a health care team.

We are your partner in providing you with the best care possible.

Our coordinated care team approach fosters open and ongoing communication between our health care providers and our patients. But the best health outcomes happen when there develops an ongoing, trusted relationship between our care providers and our patients.

In everything we do, we will strive to earn and keep your trust. We hope in return, that you as our patient, will also do your part.

WE WILL:

Respect you as an individual

  • Listen to your feelings and questions to help you make decisions and set health goals
  • Explain diseases, treatments, and results
  • Keep your medical information and records private

Provide safe and qualified care

  • Provide you with your own primary care physician
  • Provide clear directions about medicines and treatments
  • Send you to trusted experts, if needed
  • End every visit with clear instructions about expectations, treatment goals, medicines, and future plans

Strive to build flexibility to schedule you with your personal physician/provider whenever possible

IN RETURN, WE TRUST YOU TO:

Be in charge of your health

  • Learn about wellness and preventing disease and make healthy decisions
  • Be honest and thorough about your medical history, symptoms, and any changes in your health
  • Tell us what medications you are taking and ask for refills during your office visit
  • Tell us when you see other doctors, the medications they have prescribed, and ask them to send a report about your care
  • Learn what your insurance covers

Be a good patient

  • Take all your medicine and follow your treatment plan, or tell us if you cannot do so
  • Respect us as partners in your care
  • Keep your appointments as scheduled, or call and let us know if you need to cancel
  • Pay your share of the Health Center visit fee when you are seen in the office

Communicate with us

  • Ask questions, share feelings, and be part of your care
  • Provide us with feedback to improve services
  • End every visit with a clear understanding of your provider’s expectations, treatment goals, and future plans
