Parents and legal guardians have the right to request dependent children’s records before 18 years by completing a signed HIPAA Release Authorization Form (in English) or HIPAA Release Authorization Form (in Spanish) with proper identification.
Please note: Certified Birth Certificates are NOT available from any of our HIM Department locations. These records must be ordered from the Department of Vital Statistics in the state where the birth was registered.
