Your family is unique and so are your health care needs. We have specially trained Certified Application Counselors at select locations to walk you through enrolling in an affordable and comprehensive health care plan through the New York State of Health Marketplace. Health insurance enrollment help is free.

What is the New York State of Health Marketplace?

U.S. Citizens and individuals with a legal immigration status (Green Card or Employment Authorization Card) must have health insurance. The New York State of Health Marketplace is the online place for New York State residents to sign-up for health insurance.

In the Marketplace, there are many health care plans offered by insurance companies to choose from, but not everyone is eligible for every plan. What determines which plans you are eligible to choose is your income and the composition of your household.

Our Certified Application Counselor will ask you to provide income documentation and information about the number and ages of people in your household to determine your eligibility. The documents you will be asked to provide are listed below. Our Counselor will then work with you to see which plans you are qualified for. From that list of qualified plans, they can help you choose the best fit for you and your family and help you through the enrollment process.

Our Counselors can provide health insurance enrollment help for individuals from 0 to 64 years of age.

Types of health care insurance plans:

Qualified health care insurance plans

Qualified health care plans are insurance plans where you pay for coverage. They are reserved for individuals and families that make over 400% of the Federal Poverty Level. The monthly premium, deductibles, co-payments, and out-of-pocket expenses will vary depending on the plan and the family size. Often you can apply to get financial help to reduce the cost of health care in the form of premium tax credits or cost-sharing reductions. Generally, there is a deadline to enroll each year, but with the COVID-19 pandemic, deadlines for qualified health care plans have been extended.

Affordable health care insurance plans

There are many affordable and comprehensive health care plan options available for lower-income individuals and families. One of our Certified Application Counselors can provide you with health insurance enrollment help to get you set up. You can apply any time for these plans, although different plans have varying waiting time periods in which your coverage becomes active.

Medicaid

Medicaid is a public health insurance program that you qualify for based on your income and household composition. It provides insurance coverage for primary care, dental, behavioral health, nutrition counseling, and specialist services. Coverage is renewed every year on the anniversary of your initial enrollment and backtracked for medical services that occurred 3 months before enrollment. Medicaid becomes active within 48 to 72 hours of getting eligibility.

Expecting a baby? Medicaid provides insurance for eligible pregnant women. Coverage includes all pregnancy-related and post-partum care for up to 90 days following delivery and access to family planning services for one year. Undocumented women will have coverage through pregnancy and up to 90 days after delivery. Our Counselors are on-site to assist prenatal patients in determining eligibility.

Child Health Plus

We are a participating provider for Child Health Plus. Parents or caregivers over the Medicaid income threshold are eligible to enroll their child in Child Health Plus. If your child is already enrolled in Child Health Plus, your child can receive health care here at the center.

Through Child Health Plus, enrolled children can receive pediatric care, labs and x-rays, immunizations, prescription drug coverage, hearing, vision, dental coverage, and more. Monthly premiums vary depending on income.

Undocumented children newborn through 18 years are eligible to enroll in Child Health Plus.

The Essential Plans

Essential Plans are New York State-subsidized health care insurance plans. They are offered to qualified lower-income individuals and eligible immigrants ages 19 through 64 years. There are four Essential Plan types depending on income and immigration status. Coverage for Essential Plans 1 and 2 starts the following month after enrollment. Coverage for Essential Plans 3 and 4 begins within 48 to 72 hours after enrollment.

Essential Plans cover medical services that health insurance plans must cover under the Affordable Care Act. Monthly premiums are determined by income and household composition.

Required documents for enrolling in health insurance

In order to determine your plan eligibility, our Counselors will need to see documents that verify your income and residency status. Please bring the following documents to registration for each member in your household:

  • New York State ID (e.g. Driver’s License)
  • Social Security Card
  • Birth certificates of household members
  • Certificate of Naturalization, if applicable
  • Permanent Resident Cards (“Green Card”), if applicable
  • Employment Authorization Card (“Work Permit”), if applicable
  • Income verification for the last 4 weeks (4 paystubs if weekly, 2 paystubs if bi-weekly)
  • Most recent Federal Tax Return, if self-employed
  • Available employer-based health insurance or other insurance information
