DESCRIPTION:Food Distribution Day for WCHC Patients enrolled in the Food is Medicine Senior Program. \nThe Food is Medicine Senior Program is a nutritional resources for individuals 60+ with chronic health conditions. To qualify\, participants must be screened by a health care provider and have a history of chronic disease such as diabetes\, high-blood pressure\, obesity and/or hypertension.
