Community Outreach

An important part of Westchester Community Health Center’s mission is to ensure that quality health care is available to everyone in our community. We are especially focused on community outreach activities aimed at reaching higher-risk and underserved populations.

We have a team of four Community Outreach Workers who work year-round in local neighborhoods. They help to organize neighborhood healthcare events, provide health education to individuals and families, and conduct home visits and wellness checks. As part of their outreach, they inform the community about the services and programs that our Center provides.

Community Health Day

Meeting the healthcare needs of those experiencing homelessness

Westchester Community Health Center maintains a Homeless Services Department devoted to addressing the medical needs of those experiencing homelessness. We operate two Homeless Shelter-Based Health Care Centers – Coachman Family Center and Grasslands Homeless Shelter – where we provide primary health care services to homeless shelter residents. These health care services are offered regardless of ability to pay or immigration status.

Services available at our Homeless Shelter-Based Healthcare Centers include:

  • Comprehensive primary care
  • Health education and prevention
  • Referrals to nutritional counseling
  • Referrals to mental health/substance abuse counseling
  • Referrals to specialty care, like vision care, hearing care, and foot care
  • Referrals to OB/GYN
  • Advocacy
  • Social work
  • Pharmacy services
Coachman Family Shelter, White Plains, NY
We provide healthcare services at the Coachman Family Shelter, a 100-room facility located in White Plains, NY, that provides housing and supportive services to adults and children.

Services for expectant women and young families

Westchester Community Health Center has several Perinatal and Infant Community Health Collaborative (PICHC) Community Outreach Workers at its main location. PICHC is a New York State Department of Health Program which seeks to identify pregnant, new, and young mothers who are not receiving prenatal care, and other resources. PICHC Community Workers go out into the community and individual homes to help women get affordable and comprehensive medical and supportive care at our centers for themselves and their children.

Wellness programs for the community

Throughout the year, we offer classes focused on helping individuals make and maintain healthy choices in their daily lives. We provide hands-on instruction in nutrition, stress management, exercise, and chronic disease management. Almost all our programs are free. Programs are open to all patients, and in some cases, also to the general public.

Our health center is a Health Center Program grantee under 42 USC 254b and a deemed Public Health Service employee under 42 USC 233(g)-(n). We are an FTCA-Deemed Facility, accredited by the Joint Commission and recognized by the National Committee for Quality Assurance (NCQA).
